| EP-93 | 92 | Archit Garg | Background: Both-bone forearm fractures require stable fixation to restore length, alignment, and rotation. Elastic Stable Intramedullary Nailing (ESIN/TENS) is a widely accepted treatment modality, particularly in pediatric and adolescent populations. However, re-fracture following ESIN, though uncommon, represents a clinically significant complication with limited evidence regarding optimal management strategies. Case Presentation: We present a case of a 13-year-old boy who sustained an acute left forearm both-bone fracture following a fall on an outstretched hand while playing football. Initial surgical management consisted of ESIN fixation with acceptable post-operative alignment. At three months post-operatively, the patient sustained a re-injury from another fall, resulting in re-fracture of both the radius and ulna with obvious deformity and significantly restricted range of motion at the wrist and elbow, though neurovascular status remained intact. Surgical Management: The patient underwent revision surgery involving removal of the intramedullary nails and open reduction with internal fixation (ORIF) using plate osteosynthesis for both forearm bones. Intramedullary nail extraction was achieved using standard techniques, and rigid plate fixation was applied to both radius and ulna. Outcomes: Post-operatively, the patient demonstrated excellent functional recovery. By six weeks, full supination, pronation, wrist, and elbow movements were achieved with no skin complications. At three-month follow-up, radiographs confirmed satisfactory bony union of both shafts. At five-month follow-up, the patient had regained good range of motion without any complications. Conclusion: This case reinforces that while ESIN is effective, re-fracture remains a challenging complication. Definitive rigid plate fixation provides superior rotational control and predictable union rates. Early recognition and individualized fixation strategies that match fracture biology with patient activity levels are essential for optimal outcomes. | 22 Feb 2026 |
